Unlike the western population hip arthritis occurs in young patient in India / Asia. Hence conserving bone becomes crucial. The BHR (Birmingham Hip Resurfacing) and BMHR (Birmingham Mid Head Resection) prosthesis is specifically developed for younger and more active patients.
This is a more bone conserving technique. This procedure involves removal of just the damaged bone surface and capping it with metal head. Hence, unlike in the standard hip replacements the head and neck of femur are not removed. Also since this procedure only caps the surface of the head, the effective size of the ball is nearly same as the normal head. The potential benefits with this design are:
However, on the other side there is not enough long-term data as compared to the standard hip replacements. The longest follow-ups reported are just about a decade, unlike 20-25 years follow-up data available for the standard hip replacement. Also this procedure is not suitable for all cases, and proper case selection is mandatory.

What is BHR?
BHR is primarily intended for people under the age of 55 who require a hip replacement. In Birmingham Hip Resurfacing a very small part of bone on the top of the thighbone is resurfaced. A metal head is fixed on the thighbone. It fits into a metal socket placed in the joint. People aged between 55 and 65 who are very active and otherwise fit may also be suitable anaesthetistsd this will be determined by their bone quality and activity level.

Traditional Total Hip Replacements (THR) invariably has a ‘metal on plastic’ bearing. These are reasonably successful in elderly people who are relatively inactive, but offer unacceptably poor long term outcomes for young, active patients often with unavoidable multiple revision surgeries and associated complications.
